Thank you for your interest in completing A Web-Based Learning Module on Advance Care Planning: Enhancing the Knowledge of Clinical and Advance Practice Nurses in Tennessee.

My name is Holly D. Dockery. I am a Doctor of Nursing Practice (DNP) student in the Adult/Geriatric Nurse Practitioner track at East Tennessee State University (ETSU). As part of my training, I am completing a DNP project that will represent the culmination of my graduate education and terminal degree. The title of my DNP project is The Development, Implementation, and Evaluation of a Web-Based Learning Module on Advance Care Planning: Enhancing the Knowledge of Clinical and Advance Practice Nurses in Tennessee.

This module will address three main topics:

  • Tennessee-specific legalities of advance care planning (ACP)
  • Communication skills
  • How healthcare professionals, such as nurses, can better implement ACP into practice

The aim of this project is to improve clinical and advance practice nurses’ knowledge of ACP in Tennessee so that the quality of life and death for the rapidly growing older adult population may be enhanced.  In order to do this, I am seeking volunteers to complete the module and associated documents (pre-post tests, personal profile/demographic survey, and program evaluation) to measure if knowledge was increased secondary to the learning intervention.

Volunteers will be required to meet eligibility criteria such as being 18 years of age or older, and holding a certification as either a clinical or advance practice nurse (including licensed practical nurses, registered nurses, and nurse practitioners). Continuing Nursing Education (CNE) credit will be provided to individuals who complete the module.

This project will adhere to all privacy, confidentiality, and ethical requirements as determined by the ETSU Institutional Review Board (IRB). You may contact me at any time via email or cell phone listed below. You may also contact my DNP Project Committee Chair, Dr. Judy Rice (riceja@etsu.edu, 423-439-4396) at any time if you have any questions.

I appreciate your time and consideration in completing this module. Please continue to the next page to proceed to the informed consent.

Thank you,

Holly D. Dockery
