Urge Tennesseans to communicate healthcare preferences before medical crisis occurs
The Tennessee End of Life Partnership is proud to announce that April 16 has been declared a Nationwide Healthcare Decisions Day.

Tennesseans are urged to communicate their healthcare preferences and execute an advance healthcare directive. We also urges those who are seriously ill to learn about the Physician Orders for Sustaining Treatment (POST) program.

“Healthcare Decisions Day exists to inspire, educate and empower the public and healthcare providers about the importance of advance care planning,” said Judy Eads, President of Tennessee End of Life Partnership.

“As a result of Healthcare Decisions Day, fewer families will have to struggle with making difficult healthcare decisions in the absence of guidance from their loved one, and healthcare providers and facilities will be better equipped to address advance healthcare planning issues before a crisis and be better able to honor patient wishes when the time comes to do so.”

Tennesseans can find information and resources to begin the conversation about advance care planning on TELP’s web page including, FAQs, and advance directive forms in English and Spanish.

Advocates for National Healthcare Decisions Day are encouraged to take to social media and share why advance care planning is important.

Welcome to Tennessee End of Life Partnership Web Site

Individuals facing serious life-threatening illness and approaching death deserve to be treated with dignity, respect and compassion and to receive care that is focused on the individual’s choices for care. Families need and deserve to receive support. To achieve their goals, individuals need to plan ahead, know their choices, make sound decisions and share their wishes with their loved ones and health care professionals.  Health Care Professionals need to be aware of the patient’s goals of care and share existing medical evidence with patients to facilitate their decision-making.

Health Care Professionals should discuss choices of end of life care with their patients who have advanced progressive chronic illness, or terminally ill.

This web site aims to educate and empower patients, families, health care and other professionals to accomplish this goal.
